Insurance Information
We accept most insurance plans as well as self-pay patients. Please be sure to bring your current insurance card and photo identification with you to each visit. Co-payments are collected at check in. Patients with an HMO insurance plan must obtain a referral prior to the visit. Self-pay patients are required to pay at the time of service. If you have no health insurance and have limited financial resources, please visit our financial screening department before scheduling a visit to see if you qualify for assistance. Please keep in mind that it may take 6 – 8 weeks for the financial screening process to be completed once our financial counselors receive all required information.
